Brawl related deaths, when everyone online seems to hate you and gang up on you, especially on the temple stage. These people annoy me greatly, because the moment I move, it's all out war against me (I usually try to get the first strike, since I didn't go online to stand still.)

Something I particularly like about Temple while using Pikachu is when someone's on the top of the stage, and you're on the bottom portion, Pikachu's Thunder attack is just tall enough to have the thundercloud hit the person above. I love killing people with it, because they don't see it coming.

I once jumped off a rock in Fallout:NewVegas and died instantly...oh, and the funny part was that it was only a 2-3ft drop. (the game was being weird that day anyhow)

Minecraft: Digging directly underneath you and falling into a pit of lava. You're pretty much screwed.

Also Minecraft: Digging into a tunnel, being surrounded by lava, and then getting ninja'd by a zombie and knocked into lava. I laughed so hard when I saw it. XD
